1. THE READY PLATFORM
READY ("READY," "we," "us," or "our") is a telehealth technology platform that connects patients with independent licensed healthcare providers. READY is not a healthcare provider, medical group, or pharmacy. READY does not practice medicine and does not prescribe medications.
Healthcare services on the READY platform are provided by an independent licensed medical group ("Telehealth Provider") that operates independently of READY. Prescription medications are dispensed by a licensed 503A compounding pharmacy ("Pharmacy Partner") that operates independently of READY.
2. ELIGIBILITY
You must be at least 18 years of age and a resident of one of the 50 US states to use the READY platform. DEEP BREATH™ is available in all 50 US states. By using our services, you represent and warrant that you meet these requirements.
3. TELEHEALTH SERVICES
The telehealth services available through the READY platform are provided by the independent Telehealth Provider. The Telehealth Provider is solely responsible for all clinical decisions, including whether to prescribe any medication. READY has no control over and is not responsible for the Telehealth Provider's clinical decisions.
A prescription is required for DEEP BREATH™. Submission of an intake form does not guarantee that a prescription will be issued. The Telehealth Provider may decline to prescribe for any clinical reason.
4. PAYMENT AND BILLING
Your payment method will not be charged until your prescription is approved by the Telehealth Provider. If your prescription is not approved, you will not be charged.
Subscriptions are billed on a recurring basis: Monthly Plan at $199 per month, 3-Month Plan at $447 every three months ($149/mo), or 6-Month Plan at $774 every six months ($129/mo). You may cancel your subscription after the first two billing periods by contacting support or through your account portal.
Your bank statement will reflect "READY HEALTH PLAN" as the descriptor. No drug names will appear on your statement.
5. NOT FDA-APPROVED
DEEP BREATH™ is a compounded medication. Compounded medications are not FDA-approved. They have not been evaluated by the FDA for safety, efficacy, or quality. Compounded medications are prepared by a licensed 503A compounding pharmacy pursuant to a valid prescription from a licensed healthcare provider.
6. LIMITATION OF LIABILITY
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, READY SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE READY PLATFORM OR SERVICES.
